Annual Updates are conducted each year.

These forms will be sent to active applicants.

Download and follow the instructions for submission 

for the program(s) you are active on.

Waitlist applicants are responsible for and are required by the policies of Greenwich Communities to:

  • Report the income and assets of each member of your household.
  • Return their update BY MAIL to confirm your interest and current income. No emails, No faxes, No drop-offs
  • Notify us within 2 WEEKS of a change in income, address or household composition.

 Annual Updates for WAITING LISTS list are due xxxxxxxx 2024.

ANNUAL UPDATE HELP SHEET

Use this guide to help answer questions about the Update Process and submit the required forms and information.

HOUSING CHOICE VOUCHER ANNUAL UPDATE FORM – Use this form to update the Housing Authority with your current income and asset information. Status Letters will be sent to applicants who complete their updates before the deadline.  Applicants who miss the deadline for any reason will be removed from the waiting list.

  • Annual Updates for the HCV waiting list must be postmarked by xxxxxx, 2024. 
  • Return the entire packet by mail as follows:

FAMILY WAITING LIST ANNUAL UPDATE FORM – Wilbur Peck – Use this form to update the Housing Authority with your current income and asset information. Status Letters will be sent to applicants who complete their updates before the deadline.  Applicants who miss the deadline for any reason will be removed from the waiting list.

  • Annual Updates for the Family waiting list must be postmarked by xxxxx, 2024. .
  • Return the entire packet by mail as follows:

SENIOR DISABLED WAITING LIST ANNUAL UPDATE FORM  – Agnes Morley & Quarry Knoll I – Use this form to update the Housing Authority with your current income and asset information.  Status Letters will be sent to applicants who complete their updates before the deadline.  Applicants who miss the deadline for any reason will be removed from the waiting list.

  • Annual Updates for the Senior Disabled waiting list must be postmarked by xxxxx, 2024. 
  • Return the entire packet by mail as follows:

STATE MODERATE ANNUAL UPDATE FORM –  Adams Gardens, McKinney Terrace I and Armstrong Court– Use this form to update the Housing Authority with your current income and asset information. Status Letters will be sent to applicants who complete their updates before the deadline.  Applicants who miss the deadline for any reason will be removed from the waiting list.

  • Annual Updates for the State Moderate waiting list must be postmarked by xxxxx 2024. 
  • Return the entire packet by mail as follows:

ANNUAL UPDATE FORM – McKinney Terrace II Waiting List Use this form to update the Housing Authority with your current income and asset information. Status Letters will be sent to applicants who complete their updates before the deadline.  Applicants who miss the deadline for any reason will be removed from the waiting list.

  • Annual Updates for the MCKINNEY TERRACE II waiting list must be postmarked by xx xx , 2024. 
  • Return the entire packet by mail as follows:

HUD FORM  90103 -VERIFICATION OF DISABILITY

This form is given to your healthcare provider to complete and return to the attention of the waiting list

PENALTIES FOR MISUSING THIS CONSENT:

Title 18, Section 1001 of the U.S. Code states that a person is guilty of a felony for knowingly and willingly making false or fraudulent statements to any department of the United States Government. HUD and any owner (or any employee of HUD or the owner) may be subject to penalties for unauthorized disclosures or improper uses of information collected based on the consent form. Use of the information collected based on this verification form is restricted to the purposes cited above. Any person who knowingly or willingly requests, obtains, or discloses any information under false pretenses concerning an applicant or participant may be subject to a misdemeanor and fined not more than $5,000. Any applicant or participant affected by negligent disclosure of information may bring civil action for damages and seek other relief, as may be appropriate, against the officer or employee of HUD or the owner responsible for the unauthorized disclosure or improper use. Penalty provisions for misusing the social security number are contained in the Social Security Act at 208 (a) (6), (7) and (8). Violations of these provisions are cited as violations of 42 USC 408 (a), (6), (7) and (8).
